Below is the Audit Assistance Spreadsheet (Itemized Deductions), personally designed by me, to input the itemized deductions on for a client.

Once the client delivers all necessary and supporting documentation pertaining to their audit, we will complete the spreadsheet.  The Audit Assistance Spreadsheet (Itemized Deductions) should NEVER be transferred to a client.

Also, all lettering placed on the spreadsheet should be in UPPERCASE, and you will need to insert and/or delete rows when necessary. 

Note: If a client is getting audited for multiple years, a separate Audit Assistance Spreadsheet (Itemized Deductions) will need to be completed for each year.
