Tax Advisors, Veteran Tax Advisors, Master Tax Advisors, and Franchisees:

Below is the Medical, Dental, Prescription, and Vison Miles Log, personally designed by me, to prove the medical, dental, prescription, and vison miles driven by the client.

E-mail the client the Medical, Dental, Prescription, and Vison Miles Log and explain to them how to complete the log using the three days of examples as a reference.  Instruct them to highlight and erase the examples and complete the Medical, Dental, Prescription, and Vison Miles Log for the year they are being audited for. 

Also, make sure you inform the client that all lettering placed on the log should be in UPPERCASE, and they will need to insert and/or delete rows when necessary. 

Note: If a client is getting audited for multiple years, a separate Medical, Dental, Prescription, and Vison Miles Log will need to be completed for each year.
